Horizon Houses and Five Rivers Housing provide permanent supportive housing for adults with chronic and severe mental illness, supported by Catholic Charities.
They provide non-medical case management services to residents.
Horizon and Five Rivers' group homes are in Knoxville, Maryville and Morristown.
